Long-Term Case Management
Poverty is a complex problem that cannot be solved in one or two visits. To combat long-term poverty, we offer long-term case management in our Self-Sufficiency Program.
Through our Self-Sufficiency Program, clients are paired with Self-Sufficiency Coordinators who will help to set goals, identify challenges, and map a course to success, all while focusing on strengths and skill-building along the way. Once needs are assessed and goals are set, clients will receive an individualized action plan focused on strengths they already have, and then will be connected to community resources and supports to achieve self-sufficiency and, ultimately, graduate from the program with long-term success.
Case Management is available to qualified individuals for up to 5 years.
